The Post-Acute CRM for Home Health and Hospice
About Us
Built for Post-Acute Teams By Someone Who Had to Unlearn Everything
ReferralFlow's founder, Dr. Joshua Newman, spent years in medicine, health tech, and enterprise software. When he moved into hospice leadership, he brought frameworks, experience, and confidence that the right system would unlock growth.
Reality had other plans.
Post-acute growth doesn't run on pipelines and conversion funnels. It runs on relationships, consistent presence, and knowing which accounts actually matter right now. And the software built for "sales" had no idea how to handle that.
Why Existing Tools Miss the Mark
Josh tried everything:
- Modern CRMs built for digital marketing, e-commerce clicks, and email sequences
- Healthcare CRMs bolted onto EMRs as an afterthought
- Enterprise platforms designed for international 7-figure deal cycles (Love you guys ❤️)
- Spreadsheets and forms cobbled together and pushed to the limit
They all failed. None were built for us. Our work is different.
Our work happens in parking lots between visits, in hallways during quick conversations, and in cars while driving to the next account. It requires speed, simplicity, and flexibility.
Those tools didn't get it. So we built one that does.
What We Built
ReferralFlow is a decision and insight engine for post-acute growth.
You can't improve what you can't see. You can't coach without data. And you can't scale if your system fights your team.
We built software that:
- Shows you what's actually happening in the field
- Surfaces the accounts and actions that drive census
- Makes AI useful (and hopefully cool) instead of annoying
- Gets out of your way when it should
- Demands your attention when it matters
Fast, clear, and built for the way liaisons and agencies actually work.
